Medical Waivers and Student Profile

Camp Forms + Student Photo

Your medical forms/waivers are now available via CampDoc. If you have not received an email from CampDoc inviting you to create an account and fill out your student’s information, please check your junk folder. If it is not there, let me know and we will resend it to you.

If you have not yet uploaded your teen’s photo to CampDoc, please do that at this time! As summer approaches, we pray for your student and use the photo as a staff to familiarize ourselves with who will be coming to our experiences this summer. If you have any trouble uploading a photo, please feel free to share it with us via email, [email protected]

Note: If you have been to an Impact 360 summer experience before, you should already have an existing account with an invite to update your information and fill out new forms.

Arrival and Departure

Flight and Travel Information for Arrival and Departure

Arrival: Sunday, June 23

Please arrive for check-in no earlier than 2:00 PM EST and no later than 3:00 PM EST

Departure: Saturday, June 29

Parents arrive at 10:15 AM EST, Students depart at 11:00 AM EST

Air Travel: *If your student will be flying to camp: they must arrive to the Hartsfield/Jackson Airport in Atlanta, GA on Sunday, June 23 by 1:00 PM EST at the latest and depart from the Hartsfield/Jackson Airport no earlier than 2:00 PM EST on Saturday, June 29.

We will have a team waiting for them at the airport to shuttle them to our campus in Pine Mountain, GA. As you book their flight, please complete this form to ensure that our team is prepared for your student.

More detailed instructions will go out to all of our students flying as we get closer to camp.

Rooming Request

We work to honor roommate requests to the best of our ability, but we cannot guarantee or promise that every request will be honored.

If your student has a roommate request, please complete this form no later than April 1, 2024.

*Please ensure all parties involved submit a roommate request. Roommate requests only submitted by one party will not be honored. If you have multiple students coming, please fill out a separate form for each student.
